How to Get Distilled Fear in Diablo 4

Nightmare Dungeons (Tier 46+)

Distilled Fear begins dropping from Nightmare Sigil Tier 46 and above. Each completed dungeon at this tier has a chance to reward Distilled Fear in the end-chest. Higher tiers give better quantities per run.

The most efficient method: craft Tier 60–70 Nightmare Sigils and run them back-to-back. At these tiers, Distilled Fear drops consistently (1–3 per run). Sigil Powder from salvaging unused sigils feeds back into crafting new ones, creating a sustainable loop.

Infernal Hordes

Infernal Hordes (introduced in a later season) award Distilled Fear in the Spoils of Hell chest at the end of each run. The quantity scales with the number of Infernal Compasses used and the aether collected during the event.

Crafting Stygian Stones from Distilled Fear

Distilled Fear is combined with other materials at the Alchemist to create Stygian Stones — the actual summoning item for Tormented bosses.

The exact recipe changes between seasons, but typically requires:

  • 3–5 Distilled Fear
  • Other boss materials (Mucus-Slick Eggs, Shards of Agony, etc.)
  • Minor crafting reagents from the Alchemist
